  The staff at Union County Orthopaedic Group are caring, helpful and concerned about you. They are specially trained to assist your needs and have your best interest in mind. Our staff works closely with the physicians to ensure that patient care is coordinated at all levels. You may contact our staff members at any time during working hours. Our office is composed of receptionists, medical secretaries, surgical schedulers and nurses who are there to serve you. If you have complex issues or feel that an issue requires additional attention please ask to speak with our office manager.

  • Quickly and conveniently obtain forms from our office. You may bring or mail your completed forms to the office at the time of your visit. To view the forms listed below, you will need Adobe Reader. (You may download Adobe Reader for free at www.adobe.com).

  • Your initial visit
  • In order to make your first visit as efficient as possible please arrive 15 to 20 minutes before your scheduled appointment time. Downloading the forms above and completing them out ahead of time will also expedite the process.
  • Finally, it is important to bring the following:
  • Driver’s license
  • Insurance card
  • Referral form
  • Contact information of your primary care physician (if applicable). In particular, his or her name, address, phone number
  • Copies of all reports from any prior study or medical treatment rendered regarding the injury for which you are being seen
  • Copies of x-rays regarding the injury for which you are being seen

  • Our office is open to serve you Monday through Friday from 8am to 5pm. Please call for specific doctor’s appointment schedules.
  • After Hours Care
  • Our physicians our available for emergent consultation 24 hours a day, 7 days a week. Please call (908) 486-1111 for medical emergencies which occur after normal office hours. You will be connected to our pager service and one of our doctors will return your call.
  • Referrals
  • If your insurance requires a referral prior to seeing a specialist you must obtain this referral from your primary care physician (PCP) before you can be seen by us. Please contact your insurance company if you have any questions regarding this policy. Unfortunately, we will be forced to reschedule your appointment if this referral is not available at the time of service.
  • Patient Financial Responsibility
  • You, as the patient of Union County Orthopaedic Group, are responsible for all the fees you incur. We accept most insurance plans and will file your claim for you. It is very important that you check with your insurance plan ahead of time to confirm if your visit is considered in network or out of network. You are responsible for all co-payments and/or balances as required by your insurance plan. All co-pays are due at the time of service. For your convenience we accept cash, personal checks, Visa, Master Card, and Discover. You must bring your insurance card to ALL visits. A patient who is under the age of 18 must be accompanied by an adult who is responsible for his/her deductible. This applies to all visits — we cannot see minors without an adult present.
